How to be a patient: Elko Family Medical and Dental Center accepts new patients. It is appreciated if you call in advance for an appointment. The Medical Center welcomes most private insurance, Medicaid, Medicare, Women’s Health Connection and Nevada Check-Up. A sliding fee schedule for low-income patients who do not have insurance is available. Identification and income information is needed to enroll in the sliding fee program.
Appointments are strongly recommended; some sites do see walk-ins for emergency needs. Call the medical center to check on walk-in availability.
Prescription Refills: Ask your pharmacist to fax your request for prescription refills to the doctor; this will speed up your refill process.

Felix DeGuzman, MD, FACP is a board certified Internal Medicine physician. He received his medical degree from the University of Santo Tomas Faculty of Medicine and Surgery in Manila, Philippines. He completed his Internal Medicine residency at Harlem Hospital Center, an affiliate of the College of Physicians & Surgeons of Columbia University. He completed one year of Infectious Disease Fellowship at the University of Missouri-Kansas City. He is a Fellow of the American College of Physicians. He is currently a Clinical Assistant Professor of the University of Nevada, School of Medicine and teaches medical students at the Carlin and Elko clinics. He enjoys fishing, camping, swimming and snorkeling.
Taylor Crowther, PA-C, is a board certified Physician Assistant. He received his Master of Medical Science in the Physician Assistant Program at Mercer University in Atlanta, Georgia. He has a background in exercise, nutrition, and weight loss counseling.
Jocelyn “Joy” DeGuzman, MD, is a board certified pediatrician. Dr. “Joy” as she is fondly called by her patients, received her medical degree from the University of the East—Ramon Magsaysay Memorial Medical Center. She completed her residency in Pediatrics at the University of Illinois at Chicago—Christ Hospital Medical Center and her fellowship in Pediatric Endocrinology at Schneider Children’s Hospital/Long Island Jewish Medical Center in New York. She is a member of the Northeastern Nevada Head Start Health Advisory Board and the Maternal and Child Health Advisory Board of the State of Nevada. Her special interests include children’s growth and development, autism and neurodevelopment disorders and immunizations. She enjoys camping and the outdoors with her husband and three daughters.
Meenakshi “Meena” Patel, DMD, received her Doctorate in Dental Medicine from the University of Medicine and Dentistry in New Jersey. She completed a General Practice Residency at St. Barnabas Hospital in Bronx, NY.
Bryce S. Putnam, DMD, received his Doctorate in Dental Medicine from University of Nevada Las Vegas, School of Dental Medicine.
