How to be a patient: Sierra Family Health Center accepts new patients. It is appreciated if you call in advance for an appointment. The Medical Center welcomes most private insurance, Medicaid, Medicare, Women’s Health Connection and Nevada Check-Up. A sliding fee schedule for low-income patients who do not have insurance is available. Identification and income information is needed to enroll in the sliding fee program.
Appointments are strongly recommended; some sites do see walk-ins for emergency needs. Call the medical center to check on walk-in availability.
Pharmacy: Sierra Nevada Health Center houses an in-house pharmacy for your prescription needs. Medications are available at discounted prices for all NVHC patients. Most insurances and the sliding fee scale are accepted. For information, click here.
Prescription Refills: Ask your pharmacist to fax your request for prescription refills to the doctor; this will speed up your refill process.
Additional Information: Sierra Family Health Center opened its doors in the capital of Nevada in 1995, and moved to it’s new location in August, 2012.
Providers at Sierra Nevada Health Center:
Dr. Ali Shahin, MD, is the regional clinical director of Sierra Nevada Health Center in Carson City. He is board certified in Family Medicine and Sports Medicine. Dr. Shahin received his medical degree from Heinrich Heine University in Duesseldorf, Germany. He completed his Family Medicine residency at Doctor’s Hospital of Michigan in Detroit, where he was Chief Resident during his final year. He did a fellowship in Sports Medicine at the University of Nevada in Reno.
Carla Cordova-Eduave, PA-C, is a board certified Physician Assistant. She received her Bachelor’s in Clinical Health Services in the MEDEX Northwest Physician Assistant Program from the University of Washington, School of Medicine.
Rebecca Kemp, PA-C, is a board certified physician assistant. She graduated PA school at Western University of Health Sciences. She has worked in Nevada as a Physician Assistant since 2000.
Dr. Cesar Udani, MD, is a board certified Family Practitioner. Dr. Udani received his medical degree from Far Eastern University-Nicanor Reyes Medical Foundation, Manila, Philippines. He completed his Family Practice residency at McLaren Regional Medical Center in Flint, Michigan.
Leslie Thompson, MS, PA-C, has been with Nevada Health Centers for over 13 years. Her approach to health care makes certain that the patient is the driving force on the road to optimal health. Leslie received her Master’s of Science in Nutrition from the University of Washington. She continued her education at the University of Utah in Physician Assistant studies. She also serves as the Clinical Director of EHR Applications for NVHC helping to assure that the electronic software systems benefit Nevada Health Center’s patients, providers and staff.
Tatiana Warner, PA-C, is a certified physician assistant. She has practiced medicine as PA for over 10 years. Tatiana received her Masters at the University of California, Davis in Physician Assistant studies. She enjoys theater, reading, walks, gardening, and swimming.
